Zion shall develop services that provide:
The program will provide weekday options for an adult with developmental disabilities of all ages, teach members/consumers based on the ISP outcomes and provide the Support Coordinator the teaching strategies for the outcomes within ten days of initiating service. DTA program is considered a habilitation and provides for the personal care needs of the member.
The Objective and goal are to enhance and foster self-esteem, increase social and community safety skills, initiate development of new daily living skills, decrease or eliminate behaviors that are most challenging and disruptive to positive relationships. Zion Compassion Care, LLC (Zion) shall actively involve the consumer in developing a monthly activity schedule. A copy of such a schedule will be given to the client and the family, displayed in the facility and filed up. Zion shall maintain an accurate record of the number of hours that each member/client spends daily in the facility by keeping a log of entry and exit at each point daily, and monthly progress reports will be sent to the Support Coordinators.
The program will obtain information and carry out a pre-service orientation within the facility, intimating how the program intends to meet the ISP outcomes. Zion shall maintain daily records on file as proof of the number of hours worked by each direct service staff providing direct service to members. Each timesheet, equivalent document, or data system shall contain the original signature or other independent verification of the member/member's representative after service delivery confirming the hours worked. Zion will provide adequate support to enhance program activities using agency vehicles and personal staff vehicles. Referrals may come from the Division.
Zion shall provide specialized sensory-motor, cognitive, communicative, social interaction, and behavioral training to promote skill development for some portion of a 24-hour day after school during the year. This program will provide weekday options for school-aged children with developmental disabilities (ages 3-22), teach the consumer based on the ISP outcomes, and provide the Support Coordinator the teaching strategies for the outcomes within ten days of initiating service tracking data and provide a monthly progress report. The Objective and goal are to promote self-esteem, increase social and community safety skills, initiate development of new daily living skills, healthy eating and exercise training, computer training.
Zion Compassion Care, LLC (Zion) will actively involve the consumer in developing a monthly on-site/community-integrated daily activities schedule. A copy of such a schedule will be given to the client and the family, displayed in the facility. Zion will maintain an accurate record of the number of hours that each member/client spends daily in the facility by keeping a log of entry and exit at each point daily (Computerize Clock-In System). The program will obtain information and carry out a pre-service orientation within the facility, intimating how the program intends to meet the ISP outcomes.
Zion shall maintain daily records on file as proof of the number of hours worked by each direct service staff providing direct service to members. Each timesheet, equivalent document, or data system. Shall contain the original signature or other independent verification of the member/member's representative confirming the hours worked after service delivery. Zion provides effective support to enhance program activities, using agency vehicles and personal staff vehicles if needed. Zion Shall provide an opportunity for the member to develop skills that lead to meaningful days and promote the member's and their family's vision of the future.
Day treatment and training children's summer program is designed to provide specialized sensory-motor, cognitive, communicative, social interaction, and behavioral training to promote skill development for some portion of a 24hr day. This program will teach the consumer based on the ISP outcomes and provide the Support Coordinator the teaching strategies for the outcomes within ten days of initiating service. The program includes organized activities, a field trip to educational and cultural enrichment, and service will not be provided when the client is hospitalized.
Zion ensures that the provides specialized sensory-motor, cognitive, communicative, social interaction, and behavioral training for some portion of a 24-hour day during the summer period. The goal is to enhance and foster self-esteem, increase social and community safety skills, initiate development of new daily living skills, decrease or eliminate behaviors that are most challenging and/or disruptive to positive relationships. Zion program shall actively involve the consumer in developing a monthly activity schedule. A copy of such schedule shall be given to the client and the family, displayed in the facility, and filed up.
Zion will record the number of hours that each consumer spends daily in the facility by keeping a log of entry and exit at each point daily, and monthly progress reports will be sent to the Support Coordinators. Zion shall document a request or referral and schedule an interview with the potential consumer's family. The program manager will obtain information and carry out a pre-service orientation within the facility, intimating members and representatives on how the program intends to meet the ISP outcome. Zion Compassion Care, LLC (Zion) will provide everything necessary to support the program activities, using agency vehicles and/or staff personal vehicles.
